Mounjaro Week One: A Calm Food Checklist for Your First Seven Days

A calm first-week eating guide for Mounjaro: protein-first small meals, fluids, fibre, what the SURMOUNT-1 trial and NICE guidance actually say, and what to do when you can barely eat. Education only — dosing belongs to your prescriber.

Why does week one need a plan at all?

Because the medication quiets appetite before it teaches judgement. When hunger stops prompting you, it becomes surprisingly easy to eat 600 calories of beige snacks a day and almost no protein — and under-eaten protein weeks are where muscle goes. On any fast weight loss, a meaningful share of what comes off can be lean tissue rather than fat, and the two defences are boringly consistent across the research: enough protein, and some resistance work your body has a reason to keep muscle for.

There is a second, quieter reason: the medication window is the best habit-building opportunity you will ever get. Food noise is low, willpower is cheap, and whatever routine you practise now is the routine you will own if you ever taper off. Use it deliberately.

The seven-day checklist

  • Protein first, every meal. Eggs, Greek yogurt, chicken, fish, tofu, cottage cheese. Eat that part of the plate before the rest.
  • Small plates, no clean-plate rule. Fullness arrives early and suddenly; stopping is success, not waste.
  • Sip, don’t chug. 2 litres of fluid across the day. Nausea hates a full stomach — including one full of water.
  • Keep some fibre moving. A piece of fruit, some vegetables, oats — constipation is one of the most common early complaints.
  • Go easy on greasy and very sweet food. Most people find both sit worse than they used to.
  • Walk daily. Twenty minutes. It helps digestion, mood and the habit ledger.
  • Write nothing down except protein. One number to track in week one is enough.

What does the evidence actually say about eating on tirzepatide?

The trial that made Mounjaro famous — SURMOUNT-1, published in the New England Journal of Medicine in 2022 — followed over 2,500 adults for 72 weeks and found average weight loss of roughly 15–21% of body weight depending on dose, against about 3% on placebo. The detail that matters for your kitchen: every participant received the medication alongside a reduced-calorie diet and increased physical activity. The headline results are results for medication-plus-food-plan, because that is what was actually studied.

UK guidance says the same thing in drier language: NICE’s 2024 appraisal recommends tirzepatide together with a reduced-calorie diet and more movement, and the NHS medicines guidance for tirzepatide points patients to their prescriber for anything dose-related. None of the official documents tell you what to put on a small plate on a nauseous Tuesday — which is exactly the gap this checklist exists to fill.

For protein, the sports-nutrition research the rest of this site leans on suggests 1.2–1.6 g per kilogram of body weight per day while losing weight. What does a day of eating look like?

Honest arithmetic: the three main meals guarantee about 63g of protein, and the optional fourth act takes the day to roughly 75g. For a 75kg (165lb, about 11st 11lb) person the 1.2–1.6 g/kg range works out at 90–120g — so even a good week-one day sits below the ideal. That is expected. Week one is about defending a floor while portions are small, not hitting the target; the gap closes as the early fullness settles.

What if you can barely eat anything?

Some people’s appetite doesn’t just shrink in week one — it disappears. If that’s you: don’t force full meals. Shift to small, dense, easy things — Greek yogurt, milk, cottage cheese, a boiled egg, a protein yogurt — every few hours, and keep sipping fluids. A day of 60g of protein eaten in five small acts beats a day of two abandoned dinners. Cold food often sits better than hot; plain better than rich. This phase usually eases within the first weeks as your body adjusts.

Success in week one is not a number on the scale. It is: protein at every meal, fluids all day, one walk, and no panic about the appetite you didn’t have.
